INTRODUCTION
The Michael R. Zent (MRZ) Integrated Healthcare Center was established in January 2006 to provide comprehensive and integrated healthcare services to individuals with mental health illness in Phoenix Arizona. The integration behavioral and physical health services across the continuum of care provide a seamless care delivery system that offers patients a holistic care based on their psychosocial needs and diagnoses. Achieving a sustainable efficient and effective integrated care program requires multiple evaluation and modification of operational and administrative processes using a Plan, Do, Study, Act (PDSA) methodology.

The studies show there is confusion about the roles provided by day centers and the clinical and social needs they are able to meet (Catty et al, 2007). The study suggest that, it is probably best that people with serious mental illness, given the choice, to be able to choose the best facility for that person (Catty et al, 2017). Although, the study doesn’t strongly support day-centers in the care for people with a mental illness, the Denver House is a great resource for Tulsa. Whether a medical day center vs. a non-medical day center is better, I think that a day center for the community is a imperative part of a city. Throughout the years public health efforts have evolved in order to meet health care need of populations. Despite advances the United States continues to have high morbidity rates of individuals with chronic illness. Adding to the rates, many Americans are suffering from mental health disorders. The issue caused many research studies to focus on integrating behavioral health and primary care in order to reduce health disparities in the nation. This paper will analyze how the integration of behavioral health in primary care facilities help serve the need of our population.
Florida Assertive Community Treatment (FACT) is a program that provides comprehensive community counseling to individuals diagnosed with a debilitating mental illness. It is federally funded by the State of Florida Department of Children and Families and serves a clientele of 100 adults in the Volusia and Flagler County areas. It is a derivative of the Program of Assertive Treatment (PACT) which was developed in Wisconsin in the 1970s when deinstitutionalization was a major practice in the mental health field. This outpatient program is rehabilitation and recovery oriented. FACT is unique in that it is the only service available that offers a housing, medication, and flexible funding subsidy to enrolled individuals which help to integrate them back into mainstream civilization to be productive members of society—reducing hospitalization, homelessness, and criminal incarceration among the mentally ill.
Strategic Planning is an essential first step in the development of a result-base accountability system,” (Schilder, 2013, p. 1). In strategic planning an organization must know their goals, missions, and how to reach them. Health care facilities are in huge demand for the elderly and for people who lives in low-income communities. A person who receives good quality health care services can prevent the spread of contagious diseases. This paper will describe the need of the community, population it is serving, and a brief description of the facility.
Healthcare facilities are required to maintain licensure, certification, and accreditation in order to receive payments from federal government programs such as Medicare. Healthcare facilities must meet the minimum standards in order to operate, such as sufficient staffing, personnel employed to provide services, the quality of equipment, buildings, and supplies, and services provided, including health records. (LaTour, 2013) Medicare has developed Conditions of Participation and Conditions for Coverage, which identifies specific criteria that must be met in order to receive reimbursement from Medicare. There are many governmental agencies within the healthcare system. One of most interesting and heart-wrenching agencies is a local governmental agency called Terrell State Mental Hospital. This agency is well known in the state of Texas for its carefree healthcare. They have a longstanding relationship with the families of its patients and are a major contributor to the city of Terrell’s financial windfall. They produce over ten million a year for the city of Terrell, which allows this city to take care of its mental inpatients and make sure they have the best treatment possible.
The final item of business brought up was the vision for system integration in behavioral health. There was plenty of discussion focused on this since it is a new introduction but a much needed policy that needs to be put into place. Included in this discussion was the possibility of implementing behavioral health homes in Nebraska. Many other states have implemented behavioral health homes and some have worked out well and others have failed. One member brought up the big picture concern of behavioral health homes and how they are to take care of those with multiple morbidities since they are very complicated patients and healthcare workers tend to shy away from them. Nobody knew the answer to solve this concern but the committee planned how they should be thinking about what approach to take to make the behavioral health system more integrated. Today’s discussion about the vision for system integration was just the beginning of what is going to be handled at the next meeting.
Resources for Human Development (RHD) is a national human services nonprofit founded in 1970. The company currently oversees and supports more than 160 programs in 14 states. The programs fall under the following categories: Intellectual Disabilities, Behavioral Health, Addiction Recovery, Homelessness, Women & Children, Family Health & Counseling, Youth Development, Returning Citizens, Economic Development, Employment & Training, Veterans, Nonprofit Incubator, and Outsider Art. Within the Behavioral Health program, there are two Assertive Community Treatment (ACT) teams. The ACT Program provides community-based services to individuals with severe and persistent mental illness. The mental illness may also be accompanied by a substance abuse disorder and/or a developmental disability. The program is an outpatient agency and each ACT team has about 100 clients.
Bending the cost curve as it relates to this study is when care is organized to meet the needs of a given population. Relationships between parties, who typically may not have had traditional partnerships, were formed so collectively they can grow the strengths and diminish the weaknesses. In the case study, the Institute for Healthcare Improvement (IHI) implemented integrators that bridged the gaps and were instrumental in forging the partnerships necessary to bend the cost curve (McCarthy & Klein, 2010).
In 1965, there was a histrionic change in the method that mental health care was delivered in the United States. The focus went from State Mental Hospitals to outpatient settings for the treatment of mental health issues. With the passing of Medicaid, States were encouraged to move patients out of the hospital setting (Pan, 2013). This process failed miserably due to under funding and understaffing for the amout of patients that were released from the State Mental Hospitals. This resulted in patients, as well as their families, who were in dire need of mental health services. This population turned to either incarceration (jails and/or prisons) or emergency departments as a primary source of care for their loved ones.
The main concept of this study is to effectively display Multidisciplinary Teams and the role they have on the field of Mental Health. MDTs have been proven to be an effective format for collaboration of Mental Health Clinicians to exhibit the core concepts of wellness, resilience, and prevention for the overall health of the patient. Evidence has been shown to prove that when MDTs are being practiced in the field of study, Mental Health Clinicians exhibit enthusiasm of the skills and ideas being learned. In addition, MDTs deliver clinicians a significant amount of chances to collaborate with different clinicians to learn new skill sets and therapeutic approaches to provide exceptional and complete care for the patients. Hi Dr. Lori, on newly diagnosed pt's with Cystic fibrosis (CF) one would gear them to NIH (National Institute of Health), Mayo clinic, located in Arizona or Minnesota, were they can be part of treatment groups or pilots for medications. There are so many advances in the treatment of CF that there is an abundance of resources out there. Resources can be reached from the care managers or through joining a family group of individuals with CF. My friend, Ms. C, stated that her two sons who are now 19 and 22 years old. They have no hospitalizations in over 5 years. Statistics are numbers on pages of people we don't know, doing research for a disease. These young men would look forward to their weekly infusions of high doses of vitamins and glutathione when they were young because it made them feel better.
Heritage Oaks Hospital provides a less restrictive step-down treatment environment at three outpatient centers: Harbor Oaks in West Sacramento serves adults and senior adults; Roseville Oaks serves adolescents, adults, and senior adults; and Winding Oaks next to the main hospital serves adults and senior adults. According to course textbook, the goal of outpatient programs is to improve quality of life and to return clients to daily activities among family, peers, and the community (Varcarolis, 2013). The treatments available include psychiatric, emotional, behavioral, and substance abuse. These outpatient centers have partial hospitalization program (PHP) and intensive outpatient program (IOP).
