EDU 515 Transition Plan Chart This chart will help you organize your thoughts, create a transition plan, and transition into the teacher-leader role of your choice. For your final project in this course, you will create your philosophy of education statement and reflect on your transition plan in the Transition Plan Reflection section of the final project. Also, you will need to explain and justify the decisions you made in this transition plan. In your reflection, you will consider how building positive relationships can help you achieve your goal. Respond to the following: How would you facilitate collaboration among colleagues? Which competencies and skills are needed to make collaboration effective? How would you use data in your teacher-leader role? How does your transition plan align with your philosophy statement? Be sure that you are thinking through all the required elements as you make decisions. Also, review the Final Project Guidelines and Rubric as you start using the chart below to plan for your transition plan. For Milestone Three, you will complete this chart and begin to draft your transition plan reflection. Complete the chart by stating the desired teacher-leader role in the top row and then replacing the bracketed text with your responses.
